DENTAL BENEFIT ELECTION
United Healthcare will be the City's dental insurance provider. Select your dental plan.
VISION BENEFIT ELECTION
United Healthcare will be the City's vision insurance provider. Select your vision plan.
LIFE INSURANCE BENEFIT ELECTION
Employee voluntary life insurance coverage can be up to 5X your salary or $500,000 whichever is greater. Spouse voluntary life insurance is available for up to $150,000, but can not be more than half of the amount of the employee's voluntary coverage. Rates are based on amount and age. Accidental Death and Dismemberment (AD&D) is available for $0.03 per thousand. AD&D doubles coverage if the event of an accident. Bi-Weekly amounts will be calculated by Human Resources. Important: Any new or increase to life insurance coverage will require that the employee and/or spouse complete an Evidence of Insurability form.
